Pain Med Question

I'm on Hydromorphone Contin for CRPS. My doctor has prescribed 12 mg in the morning, 6 mg at night. Every night I start to feel lousy, I can't concentrate, my pain is worse, I feel like I need a cigarette (I quit over 2 years ago). It's driving me crazy.
