Is there something wrong with my vocal chords?

I am a live band singer. For the past 2 months I have been experiencing difficulty in controlling my voice when I sing. I find it very difficult to control my pitch especially when I am singing medium low range. I tend to crack more often and I find it hard to end a note gracefully everytime. Voice is shaky too.
