My jaw locks up and I'm only 11

I'm 11 and my jaw locks up when I wake up. it used to take like less then a min. To get it back. in but now it takes like a few min. And I'm scared it might stay locked forever. it started ever since my jaw got knocked out of place in a roller skating crash about 10 people hit me.
